The Hawaii State Department of Health Disease Investigation Branch has partnered with Anthology Research to conduct an important public health survey. We appreciate your participation in this survey. Your answers will help us better understand Hawaii State residents’ food consumption patterns, so that we may improve public health programs to serve residents and address risks related to diseases such as angiostrongyliasis (rat lungworm disease).
